Students are required to submit a student health form. In accordance with New
York State Public Health Law, UB requires that all students
(undergraduate, graduate, professional) born on or after Jan. 1,
1957 provide proof of immunity against measles, mumps and rubella.
The state also requires that all students receive information about
meningococcal disease and have made an informed decision about
whether or not to receive immunization against meningococcal
disease. UB students will not be allowed to register for classes
until they have fulfilled these requirements.
Submit your completed health form to Health Services, Michael
Hall, University at Buffalo, Buffalo, NY 14214-3077 or fax to
Instructions for what courses you register for were included in
your confirmation letter. If you have any questions regarding
registration please contact our Registrar at 716-645-1273 or e-mail
Students are required to obtain a student ID card (UB Card). The
cost of the card is $20, which will be charged to your University
account and your UB Card will be mailed directly to you. Each
request will be confirmed by verifying course registration records.
This card is not meant to be a substitute for a traditional UB
Card; it is intended to verify that you are a member of the
extended University community. If in the future you come to campus
to continue your studies or research then you must obtain the
traditional photo UB Card. You will be able to exchange this card
towards the purchase of a traditional UB Card.
Once you have your UBIT name, person
number and password you must go to Active
Your UBITName website to set up your UB email account and
gain access to UB computing resources.
