Health Home Care Management Services

Jewish Family Service of Buffalo & Erie County

The JFS Health Home offers culturally and linguistically competent services for qualified individuals that require assistance in managing their healthcare and social needs. Enrolled members work with a Community Care Coordinator whose sole purpose is to assist their member in managing their health and linking them with necessary providers.

Hope Refugee Drop-In Center provides supportive serivces to refugees who have lived in Buffalo at least 6 months and who are no longer receiving services from a resettlement agency.

Erie County Medical Center
