| Job Description: | PATIENT CARE • Complete thorough patient assessment. • Develop evidence-based treatment plans for patients. • Engage patient in a healthy therapeutic relationship. • Advocate for patient needs when necessary. • Refer patient to needed care within JRCHC or outside resources. COORDINATION • Maintain a caseload of approximately 30 one-hour patient sessions per week. • Collaborate with Providers, Interpreters, and Care Coordinators to ensure holistic care. • Submit service activity logs, weekly and monthly schedules, time sheets and mileage to the Behavioral Health Manager in a timely manner. |
| Required Qualifications: | • LMSW, LCSW, LCSW-R, MHC-P, LMFT or a LMHC with a NYS License to practice. • Ability to give full attention to what other people are saying, taking time to understand the points being made, and asking questions as appropriate. • Experience with electronic health records is a plus. • Experience working with diverse, multilingual populations highly desired. |
