| Job Description: | Niagara Falls Memorial Medical Center is looking for a Social Worker/Mental Health Counselor/Creative Art Therapist to work as part of our outpatient clinic team. This individual will be responsible for assessment of patients’ mental health functioning and the development and implementation of a plan of care to meet clients identified needs. Working on a multidisciplinary team with responsibility for performing individual, group, family and crisis interventions to stabilize symptoms and maximize functioning. Identification and clinical management of high risk – high need patients. Collaboration with community-based service providers in an effort to assure comprehensive treatment. Responsible for mental health clinical care and direct patient coordination of services. Applicants must be NYS licensed, permitted, or permit eligible.
