Job Description: Care Coordinator: HYBRID Person Centered Services Care Coordination Organization is looking for passionate Social Work Professional to work within a health home model serving people with intellectual and development disabilities. The Care Coordinator also known as a Care Manager has an overall responsibility for coordinating and advocating for all aspects of the individual’s care including, but not limited to, health and behavioral healthcare, community supports, and other services required to meet the needs of the individual. Care Coordinators and Social Workers use a person-centered, holistic approach, where each person being served has their health care and other service providers collaborate to ensure their wants and needs are met. We’re in the business of helping people and always using a strengths-based approach. We are currently seeking candidates residing in either Erie or Niagara Counties to join our team. With satellite offices in Tonawanda and West Seneca, and a unique caseload based on your location, we offer the opportunity to work in a dynamic and supportive environment. Position Responsibilities: Develops a comprehensive, person-centered Life Plan with the individual and their circle of support, as well as their entire service provider team. Effectively manage a tiered caseload, while tailoring services to individual needs. Similar responsibilities as a Care Manager. Completes enrollment and eligibility documentation. Meets with individuals in their homes, physician/provider offices, and other public places to conduct assessments and provide services. Required Qualifications: Requirements: Bachelor's degree with 2 years of relevant experience OR a Licensed Registered Nurse with 2 years relevant experience OR Master's degree with 1 year of relevant experience Minimum of a Bachelor's degree is required by OPWDD in NYS Relevant experience consists of providing direct care or support, coordination, and/or advocacy for anyone. Examples: Direct Support Professional, Teacher, Paraprofessional, Counselor, Social Worker, Care Manager, etc. Valid driver's license
Rate of Pay: $23.50
