| Job Description: | Provide telephonic, virtual and in-person guidance and support to individuals impacted by Alzheimer’s disease and other dementia across all eight counties of Western New York. Respond to disease related questions / situations by providing guidance, problem-solving, decision-making support, crisis assistance, and connections to local services. Conduct clinical assessments and action plans that identify needs and offer interventions based on those needs, including assistance with future planning, and in-depth education about the disease and caregiving. This position is grant funded. |
| Required Qualifications: | Master degree in social work, (Master’s in Counseling, Gerontology, or Family Therapy also accepted) + 1 year experience required |
| Preferred Qualifications: | Preferred Master degree in degree in social work, (Master’s in Counseling, Gerontology, or Family Therapy also accepted); prefer licensure where applicable (LSW, LCSW, LPC, LCPC) + 2 - 3 years experience |
| Rate of Pay: | The Alzheimer's Association’s good faith expectation for the salary range for this role is between $60,500 - $64,000 |
