| Job Description: | Gateway-Longview is hiring a Preventive Care Coordinator to coordinate services for families enrolled in the Erie County Wraparound Program. This position will also: - Oversee all aspects of Care Coordination for families enrolled in Wraparound Services. - Facilitate all Client and Family Team Meetings (CFTs) including organizing meeting logistics and ensuring all relevant participants are included. - Ensure the CFT’s integrate all involved systems including the family’s formal and informal support community to develop a comprehensive Plan of Care. - Ensure all contracted services such as mentoring, tutoring, parenting, counseling or other assigned services are delivered timely and effectively to families. - Input and update all necessary documentation in Fidelity EHR and/or CONNECTIONS including Assessments, Plans of Care, Safety and Crisis Plans and Progress Notes - Review and approve notes from contracted providers within the required time frame. Attend required meetings regarding Erie County Wraparound - Provide ongoing support to families in the Wraparound system including regular telephone and weekly face to face contact, and availability by cell phone. |
| Required Qualifications: | - Bachelor’s Degree with 4 years of experience - Valid New York State driver’s license and adequate auto insurance. - Candidates must have flexibility to meet with clients and families around their schedule, including evenings and weekends to support specific programmatic schedules, crisis intervention needs, and on-call support. Ability to flex schedule based on these needs is allowed. - Ability to work effectively with clients, families, staff and community contacts from a variety of cultural and ethnic backgrounds |
