| Job Description: | JOB SUMMARY: Provide leadership, direction and oversight of special project(s) across the agency. Directly responsible for routinely providing data, reports and implementation plans reflecting management of contract/grant and progress of project(s) development, sustainability, expansion. Manage, accomplish and monitor contract success. Address and move beyond barriers as they arise. ESSENTIAL FUNCTIONS: • Exemplify Catholic Charities' mission, vision, guiding principles and policies and procedures • Promote and demonstrate integrity and ethical performance, consistent with the Catholic Charities’ Corporate Compliance Program, Code of Ethics and other professional standards, i.e. NASW, APA. • Coordinate project development and budget within project timelines. • Document project phases (milestone tasks, person responsible & timeframes), summaries and reports to management and external authorities. • Recruit, orient, supervise and evaluate the project assistant; Conduct required provisional and annual performance evaluations of project assistant; maintain performance accountability and implement corrective supervisory action as warranted. • Ensure that all project-involved personnel have the tools they need to meet projected milestones. • Routinely monitor and analyze project data to ensure project diligence with comprehensive tracking via central database. • Collaborate with both internal and external key stakeholders to coordinate project management activities, resources and information, as well as routinely provide project progress reports. • Assist in developing and implementing continuous quality improvement (CQI) processes. • Assist in identifying and implementing best practices that maximize productivity and cost-efficiency. • Collaborate with Contract Manager regarding NYS trends and initiatives from DOH, OMH, OCFS, and OASAS in regards to children, adults and families as Contract Manager is aware of potential opportunity for project expansion. • Perform other duties as requested by Supervisor |
| Required Qualifications: | REQUIRED EXPERIENCE, KNOWLEDGE, SKILLS AND ABILITIES: • Demonstrated agility, initiative, innovative, to ability to work independently and efficiently (effective organizational and time management skills), make data-driven decisions, produce results, and meet deadlines. • Demonstrated experience with assessing and defining the purpose/scope of a project, and leading the transition of innovative ideas from pilot to implementation. • Demonstrated ability to organize, analyze and report data. • Demonstrated experience with value-based healthcare delivery models. • In-depth knowledge of electronic health records and data bases such as 10e11, PSYCKES and HEALTHeLINK. • Working knowledge of clinical quality metrics such as HEDIS measures. • Demonstrated experience with best, evidence informed and evidence based practices. • Demonstrated effectiveness with leading teams and engaging with cross-functional systems & leadership to cultivate strong team collaboration. • Fluent in various computer applications (e.g. Word, Excel) and electronic database systems; experience with graphic reporting applications preferred. • In-depth familiarity with WNY community and continuum of human services (health, behavioral health and social services) systems, organizations, operations and resources. • Evidence of exceptional interpersonal and communication (oral and written) skills. |
| Preferred Qualifications: | MINIMUM REQUIREMENTS: EDUCATION: Master’s degree in Social Work, Mental Health Counseling, Psychology, Nursing, Public Health, Health Administration, Business Administration or related field from an accredited program. Bachelor’s degree in related fields with equivalent experience. Clinical experience preferred. |
