Job Description: Preventive Services Worker assesses, monitors, and plans for the safety, permanency and well-being of children at risk of out of home placement due to child abuse or neglect. Responsible for exercising considerable initiative and independent judgment in the performance of duties. Incumbent discerns and utilizes the appropriate intervention or referral, honoring family-identified needs as well as court/LDSS identified needs. • Provide Child Welfare services (working in a home-based and community setting) to families with children at risk of placement in foster care that have been referred by ECDSS through our Preventive Services program following Preventive Regulations (18 NYCRR Part 423 & Part 428) • Initiates contact with the client family in their home, accurately assesses service needs, links and refers, and follows up • Obtains information to evaluate family needs, giving consideration to family voice • Develops treatment plans based on accurate assessment, court orders, and needs identified by the family and CPS investigation • Provides direct services to families • Utilizes a Family Systems / Solution Focused / Trauma Informed Care approach to provide full range of services addressing family needs • Continually assess and alleviate safety risks via creation and implementation of a safety plan and related services • Sensitivity, knowledge and understanding of working with Disproportionate Minority Representation (DMR), immigrant and refugee families, and multiply cultured populations • Able to identify and engage natural family supports • Coordinates community resources and natural supports for accurate service delivery, assuring integration • Ability to work collaboratively with ECDSS, Family Court, schools, medical providers, social service providers, etc. • Provides objective written reports of family progress toward goals for court appearances and formal state assessments • Qualified candidates must be computer literate (able to use Connections –electronic case record, Case Management-(agency data base system) with excellent written documentation skills, as well as strong verbal and interpersonal communication skills • Participates in community affairs which relates to agency programs and goals • Implements agency policy and procedure • Takes leadership role on special agency committees • Meets quantitative expectations established by the agency and program • Performs other duties as assigned Reliable transportation required, subject to required background checks and clearances
