| Job Description: | The ROC’s model of care is designed to help individuals and families work toward self-sufficiency. This person will be responsible for conducting intakes and assessments to determine participant needs. Responsible for connecting participants with community agencies, and government and community resources. This person will develop and coordinate our Social Work Assistant Team and our Housing and Job Boards. Timely and thorough documentation and reporting of work is essential. Other duties as assigned. This person must be familiar with community programs, services, and resources in southern Western New York. Must have good communication skills, organizational skills, computer skills, ability to work as a team player. Comfortable in a fast-paced environment with multiple cases. Must be self-directed and able to work independently. Supervision will be provided. Driver’s license is required. Will be reimbursed for mileage |
| Required Qualifications: | BSW |
| Preferred Qualifications: | A BSW with experience is required for this position. An MSW is considered with interest in case management. May be asked to submit to a background check and substance abuse screening. |
