| Job Description: | SOCIAL WORKER II BSW JOB SUMMARY: Community based position supporting individuals and families with services offered through the Family Services Program in Niagara County. The position serves Niagara County by offering assistance out of both Niagara Falls and Lockport offices. Services include case management to enhance individual and family functioning as well as addressing poverty concerns, providing emergency financial assistance and advocacy, community outreach and facilitating groups. ESSENTIAL FUNCTIONS: • Conduct interviews for emergency financial assistance, assess areas of need, determine eligibility for programs and connect to other appropriate services. • Understand and follow all guidelines and documentation requirements for all assistance programs. • Maintain updated information of all community/county resources • Provide case management support to individuals and families, including assess needs and assisting with linkage to all available resources. • Coordinate and facilitate groups. Coordination includes completing paperwork requirements and maintaining positive relationships with community referral sources and collaborators. • Attend community and interagency meetings to support collaborative efforts to meet the needs of our local community and residents. • Assist with new program development to address unmet community needs. • Participate in other marketing and outreach activities as directed. • Completes all documentation and reports within required time frames. • Actively participate as team member in provision of services and office coverage. • Reliable in completing tasks and outreach as directed by supervisor and following all agency guidelines. MINIMUM REQUIREMENTS: EDUCATION: Bachelor's degree in Social Work or related field EXPERIENCE: Two years of supervised social work experience in poverty services preferred. ADDITIONAL REQUIREMENTS: • Valid NY State driver's license with reliable transportation. • Excellent written, verbal and interpersonal communication skills. |
