Job Description: ABC is seeking an experienced parent advocate/care manager in ABC’s Fast break new children’s health home program. Embedded within ABC’s mental health services, ABC’s health home program is designed to coordinate the care and provide concrete support for at-risk children (and their families) who qualify for NYS Health Home Program. As with all of ABC’s programs, the Health Home is rooted in the whole-family model, ensuring that families receive the support and advocacy to ensure the best outcomes for their children. ABC is looking for an exemplary parent advocate/care manager who is ready for the challenges and opportunities in this advocacy role. The ideal candidate will work closely with Fast break therapist to assist in completing necessary assessment and other office task as assigned by the program director. Work collaboratively across and within systems of care. Provide case management and advocacy to families Identify and address any barriers to connecting to appropriate services and helping the family to address any reluctance related to stigma, as well as concrete barriers Report to and communicate with ABC’s Lead Health Homes, NYS DOH, and other stakeholders. Required Qualifications: Candidate must have experience delivering Health Home services to children and/or similar care management work with at-risk children and families and CANS-NY certified (or willing to be certified). Candidate ideally would have experience or familiarity with Service Coordination responsibilities, exceptional time management skills and is able to adapt well to a fast-pace environment. AA or BS is required with at least 1 year of experience. Compensation based on experience. Bilingual English-Spanish required
