| Job Description: | MC Collaborative (MC2) Health Home Care Managers work closely with individuals in the Rochester, NY Community who face significant mental health and/or addiction issues. Health Home Care Managers help people obtain and maintain the supports they want in order to improve their quality of life. - MC2 utilizes strength-based, future-oriented practices that are truly person-centered. - MC2 Health Home Care Managers have smaller caseloads than other organizations. Smaller caseloads allow Health Home Care Managers to be more purposeful in the work. - MC2 is dedicated to the education and training of employees. Employees are supervised in a manner that puts learning first and have access to numerous training opportunities, including “in-house” trainings. - MC2 is continually evolving a supportive team-oriented workplace, so employee input is important. - There is strong emphasis on being in the community Health Home Care Managers serve, so there is a lot of time spent away from the office. |
