| Job Description: | Maintain a case load of 12 families, providing direct face to face contact with youth and families in accordance with the Family Voices Network and Erie County Preventive Services.; Coordinate services for families enrolled in the Erie County Wraparound Program which includes oversee the aspects of Care Coordination for families enrolled in Wraparound Services; Facilitate all Child and Family Team Meetings (CFT) including organizing the meetings logistics and ensuring all relevant participants are included. Ensure the CFT’s integrate all involved systems including the family’s formal and informal support community to develop a comprehensive Plan of Care; Ensure all contracted services such as mentoring, tutoring, parenting, counseling, or other assigned services are delivered timely and effectively to families; Input and update all necessary documentation in Fidelity EHR and/or CONNECTIONS including Assessments, Plans of Care, Safety and Crisis Plans and Progress Notes; Provide ongoing support to families in the wraparound system including regular telephone and weekly face to face contact, and availability by cell phone; Communicate with all necessary providers and supports related to the planning and care for youth/families; in addition regular contact is maintained with Erie County DSS Casework in updated and coordinating care; Other duties as assigned |
| Required Qualifications: | Master's Degree preferred with 2 years' experience or Bachelor’s Degree in addition to 4 years in related experience in public or private institutions; Flexibility to work evenings and weekends and to provide crisis support as needed; Must have an active New York State driver’s license and active insurance for purposes of travel and community work |
| Rate of Pay: | Bachelors starting rate: 17.50/hour, Masters starting rate: 19.30/hour. Offers above the base dependent on years of relevant experience |
