| Job Description: | This is a key position a grant project funded by the Administration for Community Living APS Elder Justice Innovation project. The project coordinator will work alongside Adult Protective Services in Monroe County to act as a consultant about aging services and other community supports for older adult APS clients; will conduct joint assessments with APS and help link clients with services. This position will also act as the lead care manager for other project staff who serve clients referred by APS for long term care management. |
| Required Qualifications: | BSW or MSW |
| Preferred Qualifications: | Two years of experience working with older adults. |
| Rate of Pay: | To be discussed - commensurate with similar positions in NFP agencies in region |
