Job Description: Job Description: The Transitional Case Manager is a member of the Resettlement team and is responsible for the support of clients within and beyond the initial 90-day resettlement period. The Transitional Case Manager works closely with Case Managers and the resettlement team to identify needs of clients and help them work towards self-sufficiency. The Transitional Case Manager will work with clients and partner agencies to ensure clients are connected to services by providing appropriate referrals as needed. The Transitional Case Manager must exemplify Journey’s End’s core values in their work. Primary Responsibilities: Maintain a caseload of clients seeking additional care within and after their 90-day resettlement period Provide transitional supports to clients (for example: public benefits applications, school registration, change of address, budgeting assistance, bus training, DMV assistance, clothing assistance, food assistance, health insurance) Maintain accurate and detailed case files (paper & electronic) to maintain compliance with funder and agency requirements. Work closely with Case Managers to identify those clients needing transitional support. Work with other departments at Journey’s End through client referral process Complete intake process as needed with newly referred clients. Provide culturally competent resettlement services to a diverse group of clients to support the transition to self-sufficiency. Act as a liaison with other service providers Conduct assessments with client to determine goals, barriers, resource needs and other factors related to case management, acting as a broker for services and referring clients to community resources where appropriate. Ensure availability and provide services for clients during weekly appointment hours. Other duties as assigned
Preferred Qualifications: Qualifications: Bachelor’s degree in human service or related field preferred At least one year of experience providing direct client service. Experience implementing program guidelines. Experience working with diverse populations. Ability to document work accurately and clearly through written case notes. Proficient with Microsoft Office A passion for resolving challenging problems and thinking outside of the box. Valid driver’s license and access to insured vehicle Strong communication skills and ability to work as a member of a team. Demonstrated ability to adapt to diverse and challenging scenarios involving clients, partners, staff, and service providers. Compassionate personality and general interest in human services Ability to appropriately manage stress and respond professionally to partners and respectfully to clients. Fluency in English required, proficiency in additional languages preferable but not required.
Rate of Pay: $19/Hourly
