Job Description: Gateway-Longview is hiring a Health Homes Supervisor to provide supervisory support related to care management services for children presumed to meet eligibility and requirements of New York State’s Children’s Health Home program in a trauma informed, strength based manner. This position will also: - Provide supervision and support to the Health Home Care Managers; Ensure timely completion, review and approval of all required documentation within each respective lead health home electronic medical record. In addition ensure all agency related documents are completed in a manner that adheres to Gateway’s policies and procedures. - Maintain CANS-NY supervisor certification and assure Health Home Care Managers complete and maintain necessary mandated health home training to include the CANS-NY. Ensure the care management team receives training to support health outcomes, triple aim, and the writing and implementation of the Plan of Care. - Assist Health Home Director with providing continuous QA/QI of the Health Home program including the monitoring, evaluating and reporting on outcomes, finances including the points of each care manager, and the required QA/QI of each lead health home. - Assist Health Home Director with coordinating referrals and market services for health homes. Establish and maintain cooperative relationships with referring agencies and health homes. Represent and actively participate in community meetings, webinars and lead health home activities. - Assist Health Home Director with maintaining and updating policies and procedures that align with the department of health and the respective lead health home. Ensure each care manager is fluent in the expectations of their assigned lead health home. Support the recruitment, hiring and training of all health home care managers. - Support Health Home Director in entering and managing case assignments in MAPP and maintain a minimal case load. Participate in mandatory and optional training, including but not confined to: CANS-NY family driven youth guided, trauma informed care, IT, LGBT, safety in the community work and meeting management; participate in regular supervision.
Required Qualifications: - Master’s degree in human services and a New York State Licensed Professional - A minimum of 4 years’ experience providing care management or preventative services to children and adolescents Valid New York State driver’s license and adequate auto insurance. - Solid writing and verbal communication skills, with an ability to work in an effective and focused manner. - Ability to provide supervisory support and execute effective leadership skills without direct day to day oversight. - Ability to deliver family-driven, youth-guided services that align with Health Home principles and outcomes - Experience in serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services.
Rate of Pay: 39,000-47,500/year
