Job Description: DESCRIPTION ABC's All Children's House is looking for a Caseworker for their Preventive Program: Major Duties: Conduct assessments of children and families referred by the Administration for Children’s Services (ACS), schools, other agencies and community-based organizations, or those who come in voluntarily seeking services. Work closely with the Case Planners to provide collateral services to individuals and families. Attend JHV with Case Planners and CPS at initiation of case as needed. To assist the families in accessing in-house and community-based services to address issues that place a child at risk of foster care placement and promote maximum safety, supervision and support for children and their families. This includes but is not limited to accompanying parents to programs, other service providers, domestic violence shelters, and any concrete services that may be needed. Complete referral applications and gather all required support documentation. To assist and/or model for our parents (caretakers) and children methods to achieve their service plan goals. To make required home visits including collateral caregiver homes where children are cared for during the month and collateral contacts, in order to meet ACS minimum contact mandates To request all school and medical reports; document in Connections utilizing appropriate progress note template and file in case record. Place printed copies of all-important documents in case files. Maintain a regular caseload in accordance with the program’s policies and procedures. Provide short-term crisis intervention and concrete services including advocacy, outreach and referral for designated caseload, as well as emergencies. Conduct home visits and escort program participants to appointments as needed. To carry out ongoing quarterly risk and safety assessments of the family and clearly document these assessments in Connections and in their progress notes. Bring all risk/safety concerns to the immediate attention of their supervisor. Plan, coordinate and implement groups and social activities for children, parents and families. Schedule, facilitate and/or participate in required conferences with families, community partners and ACS using the Family Team Conferencing (FTC) Model when indicated. Attend and participate in program staff meetings, as well as related agency training as appropriate. Assess if cases are appropriate for health home and begin that process. Participate in mandatory once a week individual and/or group supervision Perform other duties as assigned. Qualities Sought: Works to create and support a respectful and collegial environment. Required Qualifications: BSW/BA with related experience. Must be able to work some evenings Excellent organizational, written, and communication skills. Must be proficient in Microsoft Office. At least one year of experience working with at-risk youth and families Must have a commitment to work from a trauma-informed, strength-based background for youth and family
Preferred Qualifications: Bilingual (English/Spanish) preferred.
