Job Description: The Director of Clinical Care Coordination Services is responsible for coordinating the provision of Health Homes Care Coordination services, with a specific focus on serving high risk patients living with a diagnosis of Severe Mental Illness (SMI), patients who are HARP enrolled and the Health Home Plus population and is responsible for the following: 1. Supervises care coordinators and other direct reports. 2. Participates in the hiring and termination process of direct reports. 3. Manages aspects of program and system development for the Health Home program. 4. Conducts monthly team meetings. 5. Assures care coordination services are delivered to HARP-eligible patients in compliance with NYSDOH HARP (Health & Recovery Plan) guidelines and policies. 6. Develops and implements quality assurance activities including chart review and patient survey data for the Health Home program. 7. Conducts monthly 1:1 case specific supervision with Care Coordinators. 8. Reviews monthly services delivered to patients to assure the delivery of Health Home Core Services. 9. Assumes any associated responsibilities of the Care Coordinator (ie: provision of core services, assessment completion, and care plan completion) in times of staff vacancy. 10. Maintains and analyzes statistical and claims data for the Health Home program. 11. Ensures written policies and procedures for the Health Home program are being implemented. 12. Facilitates and participates in Case Conferences. 13. Assists in the implementation of an on-going training program for Care Coordinators. 14. Participates as a member of the Center for Care Coordination Management Team. 15. Attends initial orientation and training, required annual trainings, and other trainings to strengthen skills and improve program outcomes. 16. Adheres to policies regarding working hours, break periods and proper use of the Association’s ADP payroll and benefits system. 17. Maintains adherence to Association’s Corporate Compliance policies; promptly reports any violations to their supervisor, the Corporate Compliance Officer, or anonymously to the Corporate Compliance helpline at: (716) 541-0663. 18. Other duties as requested.
Required Qualifications: Masters of Social Work, Masters of Mental Health Counseling, or Clinical Psychologist, must possess New York State license (LMSW, LMHC, LCSW, etc.) One (1) year of experience in a supervisory or management position supervising clinicians or staff serving people living with Severe Mental Illness (SMI) and Substance Use Disorder (SUD). OR Masters in a related field (Psychology, School Counseling, etc.) with three (3) years of supervisory or management position experience supervising clinicians or staff serving people living with Severe Mental Illness (SMI) and Substance Use Disorder (SUD).
Preferred Qualifications: Current New York State license (LMSW, LMHC, LCSW, etc.) preferred.
