| Job Description: | TCD Medical PLLC is working with primary care practices in Western New York to coordinate chronic care management as well as behavioral health care management for patients. We are currently seeking Full-Time Social Workers to help us take care of our patient population with mental and behavioral health issues as well as chronic care diagnoses. Qualifications: Bachelor Degree in Social Work (BSW) with experience as a social worker in an acute, sub-acute, long-term care or health-related setting Knowledge of the discharge planning process, levels of post-acute care and a wide range of resources General knowledge of community resources to assist the patient populations Able to work within the role of a social worker on an interdisciplinary team Able to manage a high volume of psychosocially complicated cases and concurrently maintaining optimal patient/family experience Excellent interpersonal, communication, networking, and advocacy skills Our social workers function under the direct supervision of a Primary Care Physician. We are embedded in the practices' through physical presence, technology, and direct connection into the electronic health record (EHR). These professionals become a scalable, focused extension to the providers' offices we work with for our patients. |
