| Job Description: | Jamaica Hospital Medical Center currently seeks Licensed Medical Social Workers for immediate full-time opportunities within our inpatient Medical Surgical and Float units. As a member of our inpatient team, you will utilize your expertise and knowledge to provide personalized support to patients of every age. You will have a chance to augment medical care by assisting patients in making positive lifestyle changes, understanding the impact of their diagnosis, and coordinating complex hospital discharges. Additional duties will include: • Collaborating with other professionals to evaluate patients' physical and/or mental condition and to assess their discharge needs • Acting as an advocate for patients to resolve crises and provide appropriate interventions • Investigating suspected child abuse or neglect cases and initiating referrals to State Central Registry as necessary • Completing a biopsychosocial assessment of patients to gather information about their prior functioning, and support system, and evaluate their current functioning to determine a safe discharge plan. • Referring patients to community or social service programs to meet their complex needs |
| Required Qualifications: | • Master’s Degree in Social Work • New York State Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) - REQUIRED • Hospital/Medical experience preferred but new grads are welcome. • Knowledge of medical terminology • Understanding the roles of the healthcare team • Exceptional interpersonal skills • Deep understanding of the psychosocial aspects of illness and health • Knowledge of community-based organizations and resources for discharge planning • Superb written and oral communication skills • Strong crisis intervention and short-term counseling skills • Excellent time management skills and ability to work in a fast-paced work environment. Must be able to work Weekend and Holiday coverage on a rotating basis. |
