| Job Description: | Our person-centered Health Home Case Managers are dedicated to helping people with chronic or mental health conditions stay healthy and out of the emergency room. We coordinate and provide people with access to care that is available in the community, advocating for individual needs and focusing care on basics requirements and individual preferences and values. |
| Required Qualifications: | Master's Degree with a focus on Mental Health. At least 2 years case management experience |
| Preferred Qualifications: | Knowledge/experience working with vulnerable populations. Must have Driver's license. |
| Rate of Pay: | $42,000 - $48,000 signing bonus |
