| Job Description: | Position: The Social Worker is a member of the Inter Disciplinary Group (IDG) and renders to patients and families such services as are customary in social work settings in the solution of emotional, social and financial problems which interfere with optimum effectiveness of Hospice services and with the patient and family’s quality of life. Essential Job Functions/Responsibilities: • Participates in development of the initial and comprehensive care plan for each patient. Facilitates discussion of patient/family issues relating to the psychological component of Hospice. • Explores the family’s financial situation and assist families in applying for any financial assistance they may be eligible for. • Provides emotional support and other services as determined in the care plan. • Encourages open communication between family members, facilitates anticipatory grieving and assists the patient and family in their struggle towards acceptance.
