| Job Description: | This position will create and maintain a discharge plan for each youth in collaboration with the family and treatment team within the first 30 days of placement. They will also report discharge plan/changes to treatment team at every treatment team meeting held, highlight issues and establish a plan of action to overcome barriers. and establish and monitor the necessary services for the youth and family post-discharge. |
| Required Qualifications: | Bachelor's Degree required. Flexibility to work evenings to provide crisis support as needed |
| Preferred Qualifications: | Masters Degree in a clinically based area with License preferred. |
